Many Chinese students studying in the US are completely confused when they receive the medical bill after treatment, having no idea about the complete insurance claim process. Missing required documents, choosing the wrong submission channel, and failing to distinguish between in-network and out-of-network medical services may, in minor cases, prolong the review period, or in severe cases, lead to a direct rejection of the claim, leaving them to bear the exorbitant medical expenses entirely on their own.

To ensure you can smoothly get reimbursed after medical treatment, Little U has compiled this practical guide for your reference. It will walk you through step by step: the US insurance claim methods, required documents, official submission channels and key points to avoid common pitfalls.

💡 Important Reminder: This article serves as a general practical reference. All claim settlement rules and reimbursement rates shall ultimately be subject to the text of your individual insurance policy and the official documents published on the insurance company's official website.

I. Two Claims Settlement Methods

Method 1: Direct payment by medical institutions within the network

In-network institutions refer to university hospitals, clinics and pharmacies that have signed cooperation agreements with insurance companies.

  1. Before seeking medical treatment, confirm whether the clinic and the doctor are in the insurance network;
  2. Take the initiative to present your electronic or physical insurance card when seeking medical treatment;
  3. Medical institutions directly settle bills with insurance companies, and you only need to pay the Copay (fixed out-of-pocket amount) or the individual co-payment portion after reaching the Deductible (annual out-of-pocket deductible);
  4. You do not need to file a claim application on your own; just keep an eye out for the subsequent EOB (Explanation of Benefits) claim document.

✅ Advantages: No need to pay a large amount of fees in advance, eliminating the hassle of organizing materials and filing claims online, with the lowest risk of claim rejection.

Method 2: Seek medical treatment outside the network, pay out of pocket first, and then go through the insurance claim process

It is only applicable to sudden emergencies when there are no in-network medical institutions nearby. If you choose to seek medical treatment at a non-cooperative institution on your own, you generally need to pay the full amount out of pocket and submit a claim for reimbursement by yourself afterwards.

⚠️ Important Reminder: It is not recommended to proactively opt for this option unless it is an emergency! For out-of-network medical visits that are routine, without a referral and not for emergency treatment, the reimbursement standard will be significantly reduced; in some special scenarios that do not meet the policy agreements, the reimbursement ratio is only 3%-8%, which can only provide a small subsidy, and most medical expenses need to be borne by the patient themselves.

II. Advance Claim Settlement | Complete List of Compliant Reimbursement Documents

When you need to file a claim on your own after paying out of pocket, please save all relevant documentation immediately after your medical visit, as many students have been unable to file claims due to lost receipts.

  1. Itemized Bill (core mandatory document, indispensable): The bill must clearly indicate the patient's name, consultation date, diagnosis code, detailed list of medical services and items, and the tax ID of the medical institution; receipts showing only the total amount will not be approved by the insurance company.
  2. Complete payment proof: card swiping receipts, credit card statements, and online payment records, which can verify that the fee has been fully paid;
  3. Screenshots of the front and back sides of the insurance card;
  4. Official claim application form of the insurance company (directly download and fill in via the official website / APP);
  5. Supporting documents: For outpatient claim, attach the doctor's diagnosis record; for prescription drug claim, additionally provide a valid prescription; for hospitalization, submit the discharge summary; for accidental injury, prepare relevant supporting evidence as needed.

III. Three official application methods and complete insurance claim process

Once the documents are ready, you may submit your claim application through any of the following channels, with the online channel offering the fastest processing speed:

Method 1: Submit online via the official website / official APP of the insurance company [Preferred]

  1. Log in to your personal insurance account and locate the "Submit Claim" entry for filing a claim;
  2. Select the claim type: outpatient, inpatient, prescription drugs, etc. ;
  3. Fill in the consultation information item by item and clearly upload all PDF documentation;
  4. Fill in the payment receiving account, confirm that the information is correct and submit;
  5. Please keep the acceptance number, and you can log in to the backend at any time to check the review progress.

Method 2: Submit claim materials via email

Please pack all the documents and send them to the official guideline-specified email address of the insurance company, with the email subject marked as: Name + Date of Birth + Claim Application. This method applies to the situation where you are unable to log in to your online account.

Method 3: Mailing of paper materials (rarely used, with the slowest turnaround time)

Fill out the paper application form, print all original documentation and mail them to the claims center; this process takes a long time and is not recommended.

⏰ Time limit reminder: Most international student insurance policies require claims to be submitted within 90–180 days after the medical expenses are incurred, and you will directly lose the claim eligibility if the deadline is exceeded. The review period generally takes 15–30 working days, and after the review is completed, the reimbursement will be issued in the form of a check or bank transfer.

IV. List of Common Pitfalls in Insurance Claims Settlement

1. When seeking medical treatment, failing to confirm whether the medical institution is within the insurance network and blindly choosing an out-of-network clinic will result in an extremely low reimbursement rate ranging from 3% to 8%;

2. Only keeping the receipt with the total amount instead of requesting an Itemized Bill will result in a direct rejection of the claim;

3. Delaying the collation of documentation for a long time, thus exceeding the valid period for claim application;

4. Erroneous claim submission for policy exclusion items such as cosmetic treatments, health supplements and pre-existing conditions;

5. Blurry documentation photos and missing information lead to repeated material supplementation, thus prolonging the review time.

Medical costs in the US are very high, and only by mastering the complete insurance claim process can you make full use of your policy benefits. Prioritize in-network providers for payment; when you have to pay out of pocket as a last resort, keep the full set of detailed documentation, file claims on time and in compliance with regulations, and try to avoid the low reimbursement scenarios ranging from 3% to 8%.
